The Health Collaborative’s Change Makers Award highlights and celebrates organizations, community groups, and/or individuals who are going above and beyond to improve the health and well-being of the Dan River Region, prioritizing HEALTH EQUITY, IMPACT, and COMMUNITY BUILDING. We will celebrate these Change Makers and their efforts on Wednesday, January 14th at the Health Summit.

Please show your support for those who continue to work so hard to improve the health and well-being of the Dan River Region by nominating them for The Health Collaborative’s Change Maker Award by Friday, November 28, 2025.

Nomination Guidelines: Nominated efforts must impact Caswell County, NC; City of Danville, VA; or Pittsylvania County; should have taken place between January 1, 2024 and November 1, 2025; and should value HEALTH EQUITY, IMPACT, and COMMUNITY BUILDING.
