Updated Model of Health Released

Health leaders and advocates, including The Health Collaborative, have long been using the County Health Rankings and Roadmap model developed in 2014 by the University Wisconsin Public Health Institute with support from the Robert Wood Johnson Foundation to better understand the social determinants of health.

After ten years of work in this area, that model has been updated, shifting the focus from social determinants of health to structural determinants of health, effectively digging deeper into the identification of the root causes, or core drivers of health in our communities. The University of Wisconsin shares that, “people and communities can structure society so that everyone has an opportunity to thrive.”

They go on to explain that, “the University of Wisconsin Population Health Institute Model of Health shows how the community conditions – where we live, learn, work and play – affect our collective health and well-being. These conditions result from the ways in which societal rules, both written and unwritten, are used to determine which communities have access to the resources needed to thrive. People who hold more power shape the rules and how they are applied based on their values and beliefs.”

You are encouraged to EXPLORE the MEASURES of how long and how well we live and what shapes these trends. Please note that some of these measures are new and others have been adjusted so not all measures align with the original measures identified in 2014.
